West Wisconsin Telcom Cooperative, Inc. Customer Agreement Form I, the undersigned, am requesting a 24-7 TELCOM Dial-Up Internet account. I confirm that I have read and understood the Acceptable Use Policy and Privacy Policy for 24-7 TELCOM, copies of which are available at http://www.wwt.net. I understand that services cannot be resold, assigned or transferred. I also understand that information regarding updates and scheduled outages will be transmitted to me via the 24-7 TELCOM e-mail address for this account. It is my responsibility to check my 24-7 TELCOM e-mail account for this information. I understand that actual download and upload speeds are not guaranteed and will vary based on a number of factors including: distance of my location from the telephone company switching equipment, condition of telephone wiring inside and outside my location, my computer configuration, network or Internet congestion, server and router speeds of the web sites you access and other factors. Uninterrupted service is not guaranteed. Service interruptions may occur for a variety of reasons including: power failures, planned outages for network maintenance and repair work, other technical or system failures. I accept full liability to pay all charges incurred in using this Internet service. I understand that I am solely responsible for all users of this account whether usage is of deliberate or negligent actions. I understand that 24-7 TELCOM reserves the right to disconnect my account if 24-7 TELCOM has reason to believe I am in any way violating any of the terms of this agreement. IN NO EVENT SHALL 24-7 TELCOM BE LIABLE FOR ANY BREACH OF WARRANTY, DIRECT, CONSEQUENTIAL, EXEMPLARY, SPECIAL, LOST PROFITS, LOSS OF OPPORTUNITY OR PUNITIVE DAMAGES WITH REGARD TO THE INSTALLATION, OUTAGE, MAINTENANCE, USE, FAILURE OR REMOVAL OF THE SERVICE. I understand that it is my sole responsibility to supply and maintain my own firewall software. I AGREE TO HOLD 24-7 TELCOM, ITS OFFICERS, AGENTS AND ANY OF ITS MEMBERS HARMLESS FROM LIABILITY ARISING FROM SPECIAL, INDIRECT OR CONSEQUENTIAL DAMAGES INCLUDING BUT NOT LIMITED TO ANY LOSS OF DATA, LOST PROFITS, LOSS OF OPPORTUNITY OR ANY OTHER LOSS WHICH MAY RESULT FROM MY NOT HAVING OR MAINTAINING SECURITY SOFTWARE AND/OR HARDWARE TO PREVENT UNAUTHORIZED ACCESS TO MY COMPUTER FILES AND PROGRAMS THROUGH THE SERVICE. I understand that some content on the Internet may be offensive and that it is my responsibility to evaluate content accessed through the service. I understand that 24-7 TELCOM is not responsible for monitoring Internet content and websites that can be accessed using the service. Subscriber must understand that 24-7 TELCOM’S Internet Services does not own, operate, administer or endorse any information, products, or services on the Internet other than 24-7 TELCOM’S identified information on 24-7 TELCOM’S Home/Web Page located at http://www.wwt.net. 24-7 TELCOM cannot and does not guarantee the availability, completeness, usefulness, accuracy, or legitimacy of all information, products and services on the Internet or passing through West Wisconsin Telcom's Network and/or servers. Subscriber agrees that using 24-7 TELCOM’S Internet Services and the Internet is solely at the subscriber’s own risk and is subject to applicable local, state, federal or International laws and regulations. I grant 24-7 TELCOM a right-of-way easement to construct, operate, maintain a telecommunications line or system on my premises and in or upon all streets, roads or highways abutting the above premises, and also the right to cut or trim trees and to bury cable necessary to the efficient operation of the system. I certify that I am at least eighteen years of age and that I have read and understand this letter of agency and that I am authorized to change telephone companies for services to the telephone number(s) listed above. I am further authorizing 24-7 TELCOM to obtain billing information, customer service reports, and to obtain reports from credit references or credit agencies. |
